Every game I've played my defence lines have failed, I've done them according to the guides that are out there, after ending a turn and the lines form I can clearly see they're fully functioning as I want them to, however after another end turn, some lines are broken for absolutely no reason, while other lines are crossed by enemy troops like they don't exist.

What's going on here?

You mean that they've been attacked... right??

If they appear crossed by enemy units it's a bug. They've been attacked as well but somehow, the game not always gets that done right XD

They weren't attacked. Lines were crossed like they didn't exist, city was taken, no line broken despite them going straight through it.

Movement priority? He attacked your city before you put up the wall? Only logical thing I can think of. Have had it happen to me, kinda like being TBed. I've also had walls fail as well, but I just think that's because I position my troops as far apart as possible, so I think the program determines during the turn change that they are to far apart to keep the wall up.

Nope, walls were up long before any attack was made.

There was no hole, it's was a stout defensive position.

Your defensive lines can be broken for the following two reasons.

1) You did not wall properly
2) Your enemy attacked it (So-called to break a wall in AW jargon)

Since you claimed your wall are seemingly built, possibility 1 is excluded. I assume that your enemy broke it (by attacking) in accordance with your description.
